What Azalais said, plus there can be  a LOT of pressure on the woman to 
continue breeding -anyway-.  There are lots of cultures which values babies, 
particularly boy babies, far far above the lives of pregnant 
women--fictional example, the a character in MZB's Darkover series who 
suddenly after giving birth to a -son-, was showered by her husband with 
gifts and treated very nicely--as opposed to his indifference when she had 
previously produced daughters--and so she walked out and joined the Free 
Amazons (or at least, that's what I remember, it's been a long time since I 
read the relevant book(s)).

After all (sarcasm) one can always buy another wife....
