I personally figured that her milk just hadn't come in yet. It's been a while since I've read HH, but I think I presumed that the baby was within a day's journey (either it was close to her home, or they travelled with her for some of the way).  Though I think it'd be some kind of torture to be travelling right after childbirth (and I don't even want to think about horseback riding then).
