Hmm - maybe medical tech is good enough that Ivan doesn't have to worry
about being crippled for life, but something like that would certainly
throw a wrench into Miles' life as well as Ivan's.  If you grow up with
one guy the handsome athletic type, and the other the fragile genius
type, and it lasts for 30 years - a reversal where Miles has to be
careful of Ivan's fragility, and Ivan (assuming Miles goes overboard
trying to compensate like usual and poor Ivan can't escape) has to try
to manipulate Miles into doing what Ivan wants rather than what Miles
wants Ivan to want - things could have gotten interesting.
