Those are copyeditor and/or writer inadequacies.
Then there are the copyeditors who use "insinuate" instead of "insert" in 
sex scenes....

Phrases I hate:

Sometime before the age of 11 (I'm over 60) I, along with my classmates, was 
thoroughly indoctrinated in the uses of to, too and two; there, their and 
they're; for nothing OR free etc.

I still can't read or hear "for free" without wincing inside but I accept 
that it has become part of the language. I'm still fighting the 
disinterested/uninterested fight and the flaunt / flout battle. I have 
crossed through the misused word and put the other above it in a library 
book - and defacing library books is something else I was brainwashed 
against at an early age.
