My perspective:

Anything related to the presentation/appearance that is not about the words,
spelling, or punctuation.

It could be page numbers, headers and footers, italics, all caps for the
first word or two of a chapter, line spacing, spacing between paragraphs,
footnote numbering/symbols, a special symbol between paragraphs that
indicates a change of scene. It could be inconsistencies in any of these.

It could also be font issues: some words in Font A and others in Font B.

If the document contains a table of contents or a bibliography, there are
rules/guidelines for presenting those.

Depending on the tool you're using, you may be able to handle these issues
at a surface level -- that is, by making sure things look right on the
computer screen. Or you might have to insert codes into the text to get
things processed correctly.
