As in a lot of warrior aristocracies, the ones really running the district would be the lord's lady wife, along with the steward, bailiff, and the village self-goernments.

Well, given he was part of the team that put Count Dono in power and
helped maintain Count Vorbretten's title, he's been critical in
supporting Gregor's favored Progressive Party. That alone would give
him at least a minor place in the history books, even if his work with
the Dendarii remained secret.

Personally I always wondered how much of Vorkosigan District's
backwardness was due to at least a little benign neglect. While Piotr
presumably did what he could, by the time of "Barrarayar" he's already
an old man, and Aral was consumed by first the Regency and then his
work as Prime Minister, while Miles was often either gone on ImpSec
missions or recuperating. Presumably some of the responsibility for
administering things fell to Cordelia, who at least sponsored
educational programs for medical students to support isolated villages
in the mountains. This would have been worsened when Aral and Cordelia
were made co-regents of Sergyar, and Miles was still harboring
delusions of remaining with the Dendarii.

Post "Memory" of course Miles had a lot more time to run the district,
as Harra points out, and after his health crisis at the end of
"Diplomatic Immunity" Gregor would likely be more inclined to use him
for investigations close to home rather than in Galactic territory,
"Cryoburn" excepted.
