Someone who suffers a stroke, or goes into dementia... this could get
rather ugly, especially if they're not really in position to share
themselves.

Gwynne: You can only share if you've had the knife prepared, so anyone
who wants to share has made their wishes clear. I think that if someone
has slipped too far into dementia, or is extremely ill, their family would
simply carry out their wishes. To let someone die without sharing, after
they've had the knife prepared and carried it ready for years, would be
seen as a far worse act; a total disregard of their final wish.
